Elevate Media Release

Elevate Energy (“Elevate”) uses media including, without limitation, photographs, videos, sound recordings, recorded interviews and written information and input to document the programs and projects we conduct. By checking the box below you, the person named below, agree that you:

• Have willingly been, or will be, photographed, filmed, videotaped, or your likeness has been or will otherwise be captured, and/or you have willingly provided, or will provide, comments and/or other input (collectively the “materials”), to Elevate (including any employee or contractor of Elevate).
• Grant Elevate permission to use any of the materials, in whole or in part, in any and all forms and formats (whether now known or hereafter developed) including, without limitation, Elevate’s publications, websites and/or social media platforms without payment or any other consideration.
• Understand and agree that any materials will become the property of Elevate and will not be returned.
• Irrevocably authorize Elevate to edit, alter, copy, exhibit, publish or distribute these materials in any form whatsoever, whether now known or hereinafter developed, for purposes of publicizing Elevate’s programs and other work, or for any other lawful purpose, provided, however, that Elevate does not change the intended meaning of any comments (written or oral) that Elevate uses.
• Waive the right to inspect or approve the finished product, including written or electronic copy, wherein your likeness and/or other input appears.
• Waive any right to royalties or other compensation arising from or related to the use of the materials.
• Hold harmless and release and forever discharge Elevate from all claims, demands and causes of action which you, your heirs, representatives, executors, administrators or any other person acting on your behalf or on behalf of your estate have or may have by reason of this authorization.
• Confirm that you are at least 18 years of age and are competent to contract in your own name.
• Confirm that you have read this release before signing below and that you fully understand the contents, meaning and impact of this release.
